The Graduate Admissions Project (GAP) is reaching a critical stage of its second phase - the re-creation of the online graduate application form - which requires support and guidance from the wider stakeholders of the project.
A draft template of the Output PDF that is generated once an applicant has submitted his or her application has been created, which would benefit from user review and feedback from administrative and academic staff alike to ensure that it meets its needs and requirements.

Once reviewed, it would be much appreciated if you could provide your thoughts within this short questionnaire, which will take no longer than two minutes to complete; all feedback will help to formulate the final version of the Output PDF.
